Client – Attorney Case Information Statement.

Please also use the questionnaire for the Case Information Statement. We ask our clients to fill out these forms to determine your income, expenses, assets and liabilities as best as you can determine. These facts are applied to law to determine as closely as possible what your rights and obligations are under the law.

Client – Attorney Case Information Form

The facts that you fill out on the questionnaire form are then inserted into a Case Information Form which is used to attempt to negotiate the case with opposing counsel and/or which must be submitted to the Court if the case is filed with the Court. The blank final Case Information Form into which we insert your facts is also available in the forms section.
